EYELID SURGERY
Reconstructive surgery involves a large variety of eyelid
malpositions and tumor excisions.
 
EYELID TUMORS
Excisions are generally performed by a Mohs' dermatologist who has specialized training in dermatopatholgy.  The diagnosis is usually established by office biopsy by Dr. Mauriello.  Once the lesion is excised by the Mohs' surgeon, Dr. Mauriello reconstructs the defect:

Dr. Mauriello established in a series of normal patients that the distance from the corner of the eye by the nose (medial commissure) to the opening of the lacrimal drainage (punctum) was 4.5 mm in the lower lid and 3.5 mm in the lower lid as shown above. He then developed a technique for reconstructing the lacrimal drain in the eyelid (canaliculus).  The "monocanalicular stent is sutured temporarily and may be removed in the office.
 
 
 
Malhotra R, Huilgol SC, Huynh NT, Selva D.  The Australian Mohs database, part II: periocular basal cell carcinoma outcome at 5-year follow-up.  Ophthalmology. 2004 Apr;111(4):631-6.  At the time of publication, this Australian study represents the largest prospective series of periocular BCC managed by Mohs  Microsurgery.  According to the study, the 5-year recurrence rates of 0% for primary tumors and 7.8% for recurrent tumors strongly suggests that Mohs microsurgery is the treatment of choice for periocular basal cell carcinomas.
 
 
 
TEARING PROBLEMS
Tearing may result from abnormal eyelid positions that may be surgically corrected.  In addition, drainage problems may result from abnormalities of the lacrimal drainage system including the nasolacrimal duct.

Orbital surgery treatment of orbital fractures and orbital tumors.
 
Socket surgery involves treatment of the soft tissues of the orbit after the eye has been removed.

Reconstructive Ophthalmic Plastic (Oculofacial) Surgery

Smaller tumors may be excised under frozen section control or with the benefit of the Mohs surgeon.

The back surface of the new lower lid is reconstructed by a flap from the inside of the upper lid that provides structure to reconstructed lower lid.
    The front portion of the eyelid requires
    a skin graft and is nourished by the upper     lid flap.
 
Reconstructed lower lid after flap is incised.
BEFORE   AFTER repair of the everted                                                           lower eyelids.  The repair                                                                         involved a small skin  incision in                                                              the outer corner of the eye.

Note everted lower lid (above, left) that required a lower eyelid tightening combined with a full thickness skin graft harvested from the skin behind the ear (above right).
